MdeModulePkg: Move CPU_EXCEPTION_INIT_DATA to UefiCpuPkg

Since the API InitializeSeparateExceptionStacks is simplified and does't
use the struct CPU_EXCEPTION_INIT_DATA, CPU_EXCEPTION_INIT_DATA become
a inner implementation of CpuExcetionHandlerLib.
